请教关于牛奶金申请的问题

带孩子长登温哥华10个月了，准备申请牛奶金了，但是表格有些地方不清楚怎么填写，请坛子里的前辈们指导下，谢谢！

我先生目前一直在中国工作与居住，这项是填写不同地址吗？

Part 3:
Enter the date you became primarily responsible for the care and upbringing of this child.
这项是否可以填写孩子出生之日起？

需要夫妻双方都签名吗？我先生不在加拿大生活，也需要签名吗？

Enter the date you became a non-resident of Canada.
Enter the date you became a resident of Canada again.
我们一家是2013年5月短登温哥华的，13年6月回国后就一直待在国内，直到2016年7月我才再来温哥华，中间9月份我又回国了一个月，10月份再带着孩子过来一直居住在这里，请问我这种情况应该怎样填写以上这几项呢？

3. 双方签名。重点是你先生同意你申请而不是他再哪里居住。

日期填写你长登加拿大的日期，即10月份定居的日期。

多谢指点！ 有一点疑问，我的长登日期不应该是按7月来算吗？ 还是说应该按孩子长登的日期？

长登日期应该填孩子的，否则稍后要查的时候会让你把多的退回去的！

按全家定居日期算，因为从那月起发牛奶金。孩子都没来，就你一个人临时呆在加拿大，不算定居。

今年报税时我填的长登日期是7月（报税资料全部是按7月成为税务居民来填写的），如果这次填的一不致，会有影响吗？谢谢！

会有影响。成为税务居民的日期应该是一样的，可能需要填写表格调整成正确的日期。

7月份登陆后就申请了健康卡、开银行帐户等，我理解的是从7月份开始就应该已经成为税务居民了，长登日期应该填7月，而孩子申请福利的日期是不是应该在RC66SCH表的中Part D-Immigaration Status中体现？

孩子不在加拿大不能申请牛奶金。另外如果你9月份离开加拿大已经超过30天的话，健康卡也已经无效了。
To meet the minimum qualifications you must:

• be physically in Ontario for 153 days in any 12‑month period
• be physically in Ontario for at least 153 days of the first 183 days immediately after you began living in the province
• make Ontario your primary home

头183天内不能离开安省30天，回国20多天没问题。
