As a sponsor, you have the responsibility to complete and sign this form. You must send us all mandatory documents required in the requested format, otherwise your application will be returned.
Procedure
1 Gather all the required documents and supporting documents (original or photocopy) of all concerned (sponsor cosigner and sponsored persons).
2 Check in the right column of boxes that correspond to material that you submit form.
3 Attach the requested fees.

Translation
If you submit documents or portions of documents in a language other than French or English:
1. You must make these documents or parts of documents translated by a recognized as a member of a professional (certified translator) or as a person authorized to act professionally in this capacity translator. You can consult the directory of members of the Order of Translators, terminologists and certified interpreters in Quebec www.ottiaq.org site. Translations by any other person will be denied.
2. You must provide:

Proof of your eligibility as a sponsor
• recent letter or email from Citizenship and Immigration Canada containing:
- Confirmation that you have met the eligibility requirements for surety or guarantor you, inviting you to submit an application for commitment to the Department of Immigration and Cultural Communities;
- Your name (person sponsoring) and the primary sponsored person;
- The name of the office of Citizenship and Immigration Canada will process the application for permanent residence being sponsored.
