Both pieces of identification should show the name and date of birth of the minor. One must have the minor’s photo on it.
Examples include:
Identity card issued by a provincial/territorial/federal government
Social services card
a Canadian driver’s licence
a Canadian health insurance card
School ID card
a copy of the biographical page of a passport/travel document, etc.
If there is information on both sides of the personal identification documents, provide a photocopy of both sides. Note: Social Insurance Number (SIN) cards, bank cards and credit cards are not acceptable pieces of identification. Format: Clear and legible photocopy. Note: The minor’s permanent resident card cannot be used as identification.
