全家计划9月份登陆，女儿7岁，一年级已经结束了，9月开学上二年级。计划在加拿大上二年级。想问问在国内需要准备哪些文件。

我想出生证明用申请时的出生公证，疫苗注射文件国内每个小孩都有一个本本记录疫苗注射情况，是中英对照的，应该可以吧。
其他的不是很清楚。请大家指点。
健康证明是什么呢？需要医院的体检证明吗？
关于学校成绩单，需要一年级中英文的成绩单吗？
家长工卡是什么？

还需要你的居住地址证明，例如电话费账单，租屋证明。
健康证明不需要，疫苗注射的记录本就可以了；
学校成绩单，带上这两年的英文成绩单；
家长不需要工卡；
出生证明不需要，有护照就可以了。
身份证明 - 就是移民纸，大人小孩都要。

NB: To change schools you must bring all original documents as well as the child/ren as if registering for the first time!
HOW do I register my child or children for school?
* Bring
o your child or children
o original documents with certified translations into English as needed
+ immigration or citizenship documents for parents and each child
+ birth certificate, family register or equivalent document, showing date of birth for each child and names of parents
+ proof of residence for the school year such as rental agreement, purchase agreement or recent property tax statement
+ immunization records
+ report cards
# age 7-12 : at least two most recent years
# age 13 -18 : all school years from age 13 or Grade 7 onward
