加国政策变动，移民计划被废止，最近收到一封邮件要我退费，要到http://www.cic.gc.ca/english/pdf/kits/forms/IMM5741E.pdf去填IMM 5741表格，但是始终就是这样的提示：

更新了adobe也没有用，还是这样的提示，到网上找，也没有找到，给fswfeereturn@cic.gc.ca写信要，也没有回应。请问哪位有这样的表格给我一份？谢谢！
还有一个问题，就是如果超过了信件中说的两个月的期限没有寄出表格，会怎么样？钱会拿不回来吗？
