关于imm5257E

帮父母申请旅游签，麻烦各位：
imm5257E在Signature 中有一个问题：
Do you consent to be contacted by cic,or an organization at cic's request,in the future?这个在填YES还是NO？
另最后的background information 1~6我都先了NO，为什么还会出现要我提交Schedule 1.这个文件有没有格式的？

这里指的是IMM5257E-2014
