收到领事馆要求补料的邮件，
“ Details of Govt Service: A newly completed Government Service Details form (enclosed). for PA, include specific job title and description of duties This must be received at this office by: 2014/10/10”
从字面看，那是要求填一个表，Government Service Details form ，而且这个表格已经附带在邮件了了（enclosed），但是我仔细看了好多次，邮件没有attach任何附件。
而且后面还有一行说明：
“If you have been requested to submit a form, please find them on our website at
http://www.cic.gc.ca/english/information/applications/index.asp”
但是这个给出来的链接，查过所有的表格，根本就没有任何一个叫Government Service Details的。
请教一下：有没有哪位朋友填过这个表格的？在哪里下载？
还有：要求补简历，是不是就是这个表格？
