关于孩子出生证明申请几个问题请教各位

Short form birth certificate($25)
Long form birth centificate($25)(needed for passport if 16 and under
Certified photocopy of registration of live birth($50)
Genealogical photocopy of registration of live birth($50)
这四个选项只选一项还是多项，选那一个，需要几份。
birth registration number 这个怎么填写
reason why certificate is requested 这个问题怎么回答？

我只要了以上2个，办护照，办签证足够了。

申请出生纸需要填写的信息，医院给你的纸上应该有啊。

其实只要long form就够了。很多地方只要long form， 但是接受short form的地方long form也可以。两个的区别就是有没有父母的姓名而已
