A Record of Employment for this period of work must be submitted to a Service Canada Centre. Select the option which applies best to your situation:
I have the Record of Employment and am submitting it to Service Canada or it has already been submitted to Service Canada (by mail or in person, or by my employer electronically)
The Record of Employment has been or will be requested from my employer and will be submitted in the near future
The Record of Employment was not issued by my employer due to bankruptcy, the employer is out of business, or the employer has moved
The Record of Employment will not be issued because my employment is not insurable (examples: I am self-employed, I control more than 40% of the voting shares of the business I work in, etc.)
I have the Record of Employment and am submitting it to Service Canada or it has already been submitted to Service Canada (by mail or in person, or by my employer electronically)
The Record of Employment has been or will be requested from my employer and will be submitted in the near future
