2. Does the student have a parent or a sibling who possess a "CERTIFICATE OF ELIGIBILITY"?
If yes,
- Provide the parent or sibling's Certificate of Eligibility (or just provide the Quebec permanent code of eligible one).
- Provide proof of Canadian Citizenship of the parent whose eligibility is being used or the parent whose education was used for the sibling whose eligibility is being used.
- If the sibling's Certificate of Eligibility indicates that one parent differs from the parents on the child's Birth Certificate, please provide proof of Canadian Citizenship of the common parent to both children.
- Provide child's Long Version Birth Certificate, showing parents' names.
3. Did one Parent attend English Elementary School in Quebec before August 1977?
If yes,
- Parents must clearly indicate elementary schools attended.
- Provide child's Long Version Birth Certificate showing parents' names.
4. Did one Parent attend English Elementary School in Canada?
If yes,
- Provide letter (on official school letter head) of proof of majority of primary education in English (100%) or percentage of English and percentage of French if applicable. (Parents must have minimum of four years, except Ontario, which must have 5 years)
- Provide proof of Canadian Citizenship of parent who was educated in Canada.
- Provide child's Long Version Birth Certificate showing parents' names.
5. Is the child attending or did he or she attend the majority of English Elementary or Secondary school in English in Canada?
(PRIVATE SCHOOL in Quebec is assessed on a point system, please contact the Schools Board)

If yes,

- Provide proof of Canadian Citizenship of one parent.
- Provide child's Long Version Birth Certificate showing parents' names.
- Provide proof of the family's residence in another province (tax assessments, lease, health cards, etc.).

8. Did one of the parents complete the majority of their Elementary studies in English outside Canada and was domiciled in Quebec on August 26, 1977?
THIS ARTICLE NO LONGER APPLIES.
9. Are the parents residing in Quebec temporarily on a STUDENT or WORK AUTHORIZATION?
If yes,
- Provide VALID Work or Student Authorization of parent.
- Provide child's Long Version Birth Certificate showing parents' names (must be in English or French) and Immigration papers linking the whole family.
10. Is the family here on Diplomatic status?
If yes,
- Provide the ORIGINAL letter of Protocol of parent and ORIGINAL letter of attestation for child/children from Protocol Quebec
- Provide child's Long Version Birth Certificate showing parents' names (must be in English or French).
N.B. #9 & 10 Temporary Stay
If the parents status in Canada changes from Temporary Stay to Permanent Resident the Temporary Stay Certificate of Eligibility
cannot be renewed.
