An association between fetal arrhythmias and maternal use of cocoa butter
Objective
To demonstrate the association between isolated fetal premature atrial contractions (PACs) and the maternal use of cocoa butter lotion (CBL) to prevent stretch marks.

Study design
From 2000 through 2005, 294 mothers received fetal echocardiography (fecho) for fetal arrhythmias detected at routine office auscultation. At the time of fecho, the mothers were asked about their perception of fetal activity, their use of caffeinated food products (CFP) and/or CBL. Fecho measurements included definition of the arrhythmia by doppler and m-mode techniques and measurement of the excursion of the flap of the foramen ovale (septum primum) into the left atrium (LA). A redundant septum primum (RSP) was defined as tissue that moved more than ½ the way to the far LA wall. All the studies were performed by one sonographer. Tachy- and brady- fetal arrhythmias were excluded from this study.

Results
255 fetuses with isolated PACs were found to have RSP. 181/255 (71%) were considered either very active or more active than a previous gestation. 110 (43%) used both CFP and CBL. 132/255 (51%) used CLB only. 219 mothers had no ectopy after being asked to stop CFP and CBL use. 6 mothers continued to have ectopy on follow-up. An additional 38 fetuses showed no ectopy at fecho as they had be told to stop CFP and CBL prior to the fecho.

Conclusion
Active fetuses have a high incidence of RSP that can touch the LA wall with fetal breathing. Fetuses with RSP in hearts sensitized by maternal oral or topical cardiac stimulants have an increased risk of isolated PACs. Stopping oral and topical substances containing cardiac stimulants can end isolated PACs.
