本周终于收到了Eligibility Letter，从2月开始办理，历时5个半月！今天着手办理报纸广告事宜，希望接下来都顺利！谢谢“我心飞过”，谢谢Charles

2） 表格里有Conmissioner签字的地方，什么人可以做Commisioner？

2016.05.09 去Directeur de l'etat civil交了中国出生证和改名表格，收到Attestation de depot de documents au bureau du directeur de l'etat civil, documents pour une insertion d'acte etranger
2016.05.16 信件收到acknowledgment of receipt of your request for preliminary analysis for a change of name application
2016.07.13 收到大信封 insertion de votre acte de naissance au registre de l'etat civil du Quebec 以及表格和报社登载改名通知的要求
当天联系两个报社寄去支票及申请
2016.08.11 收到gazette的改名通知，当天去directeur de l'etat civil递交最后一步申请及所有材料，工作人员说要等90个工作日（4个月）
2016.08.29 收到信件，需要additional elements required to study your change of name application，需要详细解释改名原因
2016.09.14 打来电话，需要提供testimony和其他文件
2016.11.29 收到DECISION Concernant la demande de changement de nom
2017.01.11 打电话directeur de l'etat civil被告知改名证明寄错地址
2017.01.18 收到改名证书certificat de changement de nom
2017.02.01 Citizenship Canada 发邮件给我说收到公民证申请
2017.03.02 收到新名字的公民证
2017.03.07 新名字护照到手

1. Application for insertion of Act of Civil Status outside Quebec.(Nov 2nd 2015)

2. Dec 10 received a call from Etat civil asking for a photocopy of Michael’s citizen certificate

3. Dec 11th fax Michael citizen certificate to Lucie Richard at 418-643-2864

4. Dec 14th received a letter of acknowledge of receipt of preliminary analysis which is sent out on Dec 8th.

5. Preliminary analysis (Dec 14)

6. Jan 7th received a call from 418-6431447 asking for birth certificate notary from China

7. Jan 8th show up at etat civil and sent Michael’s notarial birth certificate

8. Jan 12th received a letter Impossibility of inserting a document pertaining to a birth into the Quebec register of civil status

9. Jan 15th received Eligibility to apply for a change of name, plus Insertion of your act of birth in the Quebec register of civil status.

10. Jan 19 sent out publication (108$+52$) la gazette officielle du Québec et 24H

a. Gazette, 1st publication on Feb 6th and the 2en publication is on Feb 13.

11. Application for change name on Feb 22.

12. April 5 received additional doc required “why”

13. April 15 sent the explanation letter

14. June 2, 2016 phoned le directeur de l’etat civil for changing the address. I was told that we will receive the decision made letter mi-July.

15. June 28 received the “DECISION Concernant la demande de changement de nom”

16. July 4th applied for birth certificate

17. July 21st called Etat civil, and was told that we have to wait for 30 days. It will be July 28ty.

18. 22th August received the birth certificate

我今天收到给儿子改名字的: DECISION, CONCERNANT LA DEMANDE DE CHANGEMENT DE NOM. 信中说,如果没有人去法院申请复核, 30天后将收到正式的 UN CERTIFICAT DE CHANGEMENT DE NOM.
继续等. 基本上成功!

2个插曲: 第一: 第一步在把我儿子注册进入QC 系统时, 我直接给的是国内带来的出生证原件及翻译件, 被退回来说不行. 后来重新寄去给内带来的公证件,搞定.

第二, 正式表格寄出以后,寄来一封信,说要有证据证明原来的名字不好发音. 我以儿子的名义写了封信, 描述了情况,让儿子拿去给老师和同学签名证明他的名字确实不好发音, 搞定.
改名不容易,在魁省.

我也是上周五收到的改名决定 30天后会寄来改名证书 魁省改名不容易 我历时一年时间！
期间各种插曲，差点要走法律程序了。。。受不了！ 好在抗争的结果如愿了，只是太不堪回首。在蒙特利尔改名的TX也许简单些，我是递交魁北克城，所以复杂化了。

今天终于收到了更新的公民证，历史一年零一个月完成了整个申请更名过程。

楼主你好！请问你更新公民证等了多长时间？不知你是否清楚，用更名前的公民证书和更名通知，是否可以申请护照? 谢谢！

看帖要仔细。时间在顶楼写得很详细。用旧的公民证申请护照就是旧名。护照上的新名字需要凭新的公民证去申请

昨天去护照中心办理自已的护照，问了工作人员：如果着急使用护照，可以拿更名前的公民证书，改名正式通知及出行证明 申请护照。工作人员说了公民证书变更时间很长，可以特事特办，赞一个！

今天收到:CERTIFICAT DE CHANGEMNET DE NOM, 第一时间到网上提交申请改名后的出生证!
