要你公司名义才可申请，先发传真，收到后，对方会电话确认，到时票会寄到你公司
letterhead from your company, we should have:
The number of employees working in your office or facility;
The number of invitations required;
The contact person with telephone number and e-mail address(most likely someone from Human Ressources Dept.)
by fax to the Special Projects Department (514)335-1897
