This is in reference to your application for permanent residence in Canada.
Submit the following documents within
30 days from the date of this letter.
Failure to submit these documents within 30 days may result in the refusal of your application.
For you (the principal applicant) and any dependants ages 18 and over (whether accompanying you to Canada or not):
·Complete new medical examinations if your and/or your family members medical examinations are:
- Expired; or,
- Will expire within 120 days of this letter.
New medical reports will be sent to you and family members separately.
Once completed, forward proof of your completed medical examination(s) to this office
·New and updated forms: IMM008, IMM5406, IMM5669

·New and updated original police certificates for all individuals on this application, ages 18 years of age and over (whether accompanying you to Canada or not), from each country in which they have lived for 6 months or more.

·Copy of the birth certificate issued by the hospital for you and any dependent children. For any dependent children: notarized copy of the original hospital birth certificate. For children born in the People’s Republic of China: You must submit a notarized copy of the certificate issued by the PRC Ministry of Health (If this is not available, you must submit an explanation as to why this document cannot be submitted). Kinship certificates are not acceptable.
- Copy of your updated Hukou (i.e. family register).
- Copy of your marriage certificate (if applicable).
- 2 Passport photos for each family member
·Chinese address labels for addresses in China or Taiwan
Note:
·All documents must be accompanied by a notarized English or French translation if the document is not in one of the official languages of Canada.
·Copies of documents should consist of photocopies of the originals. Notarized statements are
notadequate substitutes.
·Police certificates must be originals.
Submit
all documents in a single package by mail or courier to:

Do not submit documentation multiple times as will lead to delays in the processing of your application.
Documents should be photocopies of the originals and should be accompanied by either English or French translation. Notarized statements are
not adequate substitutes.
Please note that police certificates must be originals.
Please keep this office informed of any change of mailing address, telephone number or email address by submitting an updated IMM008 Generic application form accompanied by an updated IMM5476 Use of Representative form if you have a representative.
